避妊去勢手術は緊急手術（emergency operation）ではなく選択的手術（elective operation）です。早く実施した方がいい手術ではあるのですが、今日すぐにしなければならないというものでもありません。また見た目問題のない動物にメスを入れるのですから、麻酔事故や手術の合併症はあってはなりません。そのため、ペットの避妊去勢手術は一般的に、事前に入念な検査を行い、完全に健康であることを確認してから実施します。
しかし外猫を捕獲して避妊去勢手術を実施する場合、そうもいっていられない場合もあります。猫の健康に配慮することはもちろん重要ですが、それ以外の要素についても考慮する必要があります。「マニュアル」にはこのような考慮事項が記載されています。
●Will this patient allow himself or herself to be treated so recovery is possible from the medical issue observed? If the patient is held for treatment, will the stress of being held outweigh the benefit of the treatment?
この患者は、見つけられた医学的問題の治療のために身をゆだねてくれるでしょうか？治療のため拘束された場合、そのストレスは治療のメリットを上回らないでしょうか？
●Will this patient’s health improve without treatment?
治療しなくても、この患者の健康状態は改善されるでしょうか？
●Is there a wound or other issue that can be addressed at the same time as spay/neuter?
避妊去勢手術と同時に処置できる傷やその他の問題はありますか？
●Will this cat allow himself or herself to be trapped again? Will this be the only opportunity to alter this particular cat?
この猫は再び捕獲できますか？この猫を手術する機会は今しかないですか？
●What is the population impact of NOT spaying or neutering this patient at this time?
この患者に避妊去勢手術をしない場合、個体群にどのような影響を与えますか？
これらの懸念事項を考慮して、「マニュアル」では手術の可否について判断するためにこのようなガイドラインを例示しています。
●Surgery is usually performed when:（通常、手術を実施する場合：）
»There is mild/moderate URI（軽度/中度のURI（上気道感染症）がある場合）
»Patient is pregnant/fetuses are aborted/euthanized（妊娠している／流産している／死産している）
»Patient is thin（やせている）
»Patient has an abscess or wounds（膿瘍や傷がある）
»Patient has an unkempt hair coat（毛づやが悪い）
●Surgery is NOT performed if:（手術を行わない場合）
»Conditions are severe and anesthesia/surgery would be life-threatening（状態が悪く、麻酔や手術が生命を脅かすような場合）
»Euthanasia is considered an option based on the severity of the condition（状態が悪い場合、安楽殺も検討する）
