TNRの際に猫に施される避妊去勢手術も外科手術ですから、当然のことながら外科手術の一般的な注意事項が適用されます。「マニュアル」にはこう記載されています。
Each spay must be performed with a new individual sterilized instrument pack.
各避妊手術は、新しい個別の滅菌済み器具パックを用いて行わなければなりません。
Each neuter must be performed with an individual sterilized hemostat.
各去勢手術は、個別に滅菌された止血鉗子を用いて行わなければなりません。
※開腹手術であるメスの避妊手術は、オスの去勢手術と多少扱いが異なります。
Suture material can be taken in a sterile fashion from a reel of sterilized suture and used with a sharp sterile needle, or individual packs of suture with swaged on needles can be used.
縫合糸は、滅菌済みの縫合糸リールから無菌状態で取り出し、鋭利な滅菌針で使用するか、個包装の針付きの縫合糸パックを使用します。
In either case, suture material exposed to a patient must be discarded and not used for another patient.
いずれの場合も、患者に触れた縫合糸は廃棄し、他の患者に使用してはいけません。
Patients should be carefully monitored during surgery.
手術中は、患者を注意深くモニターする必要があります。
Medical support staff must be trained to monitor anesthesia carefully using physical parameters. Monitoring equipment such as pulse oximeters are also recommended.
医療サポートスタッフは、身体的指標を使って麻酔を注意深くモニターするように訓練しなければなりません。また、パルスオキシメーターなどのモニタリング機器も推奨されます。
モニタリングの指標として、”The Association of Shelter Veterinarians’ 2016 Veterinary Medical Care Guidelines for Spay-Neuter Programs”には
・Respiratory rate and pattern（呼吸数とパターン）
・Jaw tone（顎の緊張）
・Eye position and pupi l size（目の位置と瞳孔の大きさ）
・Palpebral reflex（眼瞼反射）
・Mucous membrane color and capillary refill time（粘膜の色と毛細血管の補充時間）
があげられています。これらのモニタリングは機器を用いなければならないというわけではありませんが、パルスオキシメーターの使用は猫の救命率を高めるという理由で推奨されています。
