Record keeping 記録
Record-keeping procedures should comply with federal, state, and local laws and regulations.
記録の手順は、連邦、州、および地域の法律および規制に準拠すべきである。(p167L)
A medical record should be prepared for each animal and should include physical examination findings, body weight, information on all drugs prescribed and administered (including doses and routes of administration), the surgical procedure performed, any abnormalities identified, and any other pertinent information regarding the animal’s condition.
医療記録は各動物について作成すべきであり、身体検査所見、体重、処方および投与されたすべての薬剤に関する情報（投与量および投与経路を含む）、実施された外科手術、確認された異常、および動物の状態に関するその他の関連情報を含めるべきである。(p167L)
Standardized operative reports may be used but should allow for additions when necessary.

避妊去勢手術は獣医療行為ですから、診療記録が必要です。日本の「獣医師法」の規定では、診療を行った獣医師はその内容について診療簿に記載し、犬や猫の場合は3年間保存しなければなりません。

混合ワクチンや狂犬病ワクチンについては手術前の接種が推奨されていますが、周術期の接種も一般的に行われていて、安全とされています。しかし、接種する、しないのどちらにしてもワクチン接種についての全責任は飼い主が負いますから、接種にあたっては飼い主の意思を尊重すべきです。
